Well-child appointments available at Forsyth Public Health

Well-child appointments are available at the Forsyth County Department of Public Health, 799 N. Highland Ave, for health assessments to enter school or for routine annual physicals for ages birth through 21 years-old.

The clinic is not able to perform sports physicals or college physicals. It is also not able to manage chronic disease or see children for sick visits. It does offer needed vaccinations and limited lab work.

To make an appointment, please call 336-703-3324. The clinic accepts Medicaid and has a sliding fee scale for other patients.
