What is Healthy Start Triad Baby Love Plus?

Just as the name implies, it is the plus for the family with a new baby, women who are in the preconception (never been pregnant) and prenatal (pregnant) stage of life, and for fathers or male partners. Triad Baby Love Plus (TBLP) Family Care Coordinators and Family Outreach Workers will assist you by making connections with community partners in Forsyth County and Winston-Salem. The program links moms, babies and dads with important health care and support services. This is a free program that is designed to improve the health of women, improve birth outcomes, and improve father/male partner involvement.

Triad Baby Love Plus provides:

  • Intensive outreach to connect women of childbearing age and their infants with prenatal and family health services
  • Case management services for up to 18 months after birth
  • Health education and training for program participants
  • Fatherhood/Male Involvement Program
  • Pre-pregnancy planning
  • Monitoring child social, emotional, and other developmental needs

Advocacy

Family Care Coordinators and Family Outreach Workers help our program participants achieve goals that are important to the family unit. We will advocate for needed services and support in accessing the following:

  • Family Planning Education
  • Medical Care
  • Child Care Resources
  • Transportation
  • Parenting Resources
  • Housing Resources
  • Mental Health/Substance Abuse Services
  • Support Services

Who is Eligible?

  • Women who are residents of Forsyth County and are 15-44 years old.
  • Children less than 18 months old
  • Fathers or male partners
